You have authorization to verify the above information verbally or in writing. You guarantee in order to induce Midas Chain Inc. to sell merchandise and extend credit to your company, the undersigned personally guarantees the prompt payment of any and all indebtedness which may be incurred by the applicant to Midas Chain Inc. In the event of any default, at any time by the applicant, Midas Chain Inc. shall be entitled to look to the undersigned guarantor(s) immediately for such payment without prior demand or notice.

In the event this account must be referred to an attorney or agency for collection any indebtedness owed by guarantor or the applicant to Midas Chain Inc., the guarantor agrees to pay all reasonable costs thereof including collection fees, reasonable attorney fees, court and expenses endured in connection therewith.

Terms & conditions 
Pricing
• All karat gold items on midaschain.com are sold by the gram except; merchandise with a genuine
  or simulated stone, gold and silver mixed items, Crystal Persuasions™, gold ball earrings and
  carded rope.
• All pricing on midaschain.com is based on the daily market price. The MSRP is triple key from
  the approximate daily cost. These prices are used as a guide for your selling reference and are
  only approximate costs.
• FINAL invoice pricing is based on the day of shipment’s daily market price, based on the 2nd
  London fixed, plus a bank fee of $10 (i.e., 2nd London fix is $1800; we will bill at $1810).

Shipping – Delivery
• All orders will be processed within 24 hours. The purchase order # can be added at checkout, if
  desired. The customer will specify all shipping instructions.
• All shipments will be shipped via UPS, FedEx, or the U.S. Postal Service, which is selected by the customer at checkout.
• Shipping and handling charges are billed to the customer.
• Shipping charges for returns and exchanges are also billed to the customer.
• All back orders will be kept for 60 days from the date of the original order, unless notified otherwise. Back orders are billed at the daily market price on the day of shipment.


General Terms
• We will always strive to have the best products and customer service, and assure that you have a great, positive and profitable relationship with Midas Chain, Inc.
• All new accounts must complete a credit application with the company’s re-sale number. This process is mandatory regardless of your requested payment terms. Open terms are available, so please speak with your customer service representative.
• MasterCard, American Express and Visa are accepted.  Additional fees may apply. Please contact customer service for additional information.
• There are no minimum orders, although minimum shipping charges do apply.
• Midas Chain, Inc. is currently unable to offer any memorandums or consignments.
• All merchandise is made using 10Kt, 14Kt, 18Kt and Sterling Silver in compliance with all U.S. laws.
• All merchandise will be stamped with “Midas” or the registered TM of the company which produced the item and will be in full compliance of the United States Law.

Returns Policy
• Prior Authorization is required for all returns. Limitations will apply.
• All items that are returned for credit must have been purchased from Midas Chain by customer.
• All warranties are void if the items were modified in anyway.
• All items with personalization are FINAL SALE and may not be returned.
• Midas Chain, Inc. is not responsible for shipping & handling costs on warranty merchandise.
Returns Process
• An authorization must be granted prior to sending in a return. Please contact customer service for details.
• When a return is approved, a “return authorization number” (RA #) will be assigned.
• Please include a copy of the invoice with the return in order to expedite the process.
• All returns being shipped must be insured and the “RA #” must appear on the outside of the box.
